Pralidoxime iodide (also known as 2-PAM iodide) is an oxime-class antidote used in emergency medicine to treat organophosphate poisoning and nerve agent exposure. It works by reactivating the acetylcholinesterase enzyme, which restores normal muscle and nerve function. It is typically administered alongside atropine
  • How it works: Organophosphates bind to and inactivate acetylcholinesterase, leading to a dangerous buildup of the neurotransmitter acetylcholine. Pralidoxime breaks this bond, allowing the enzyme to function properly again.
  • Window of opportunity: It is most effective when administered immediately or within 24 hours of exposure. If the organophosphate bond undergoes "aging" (a chemical stabilization process over hours or days), pralidoxime loses its ability to reactivate the enzyme.
  • Secondary uses: It is also used to treat overdoses of anticholinesterase medications (e.g., ambenonium, neostigmine, and pyridostigmine) commonly prescribed for myasthenia gravis
